As a skilled heath care professional, youwill deal with a wide range of health needs on behalf of frail patients as partof an MDT, ensuring the highest standards of care.

You will act within your professionalboundaries, working autonomously without immediate support and supervision,enabled by collaborative and supportive working relationships.

You will deliver safe, effectiveclinical decision-making and expert quality care, including assessment,diagnosis, clinical management and evaluation.

You will be responsible for supportingother clinicians as part of a Multi-Disciplinary Team to come up with complexplans to support frail patients with unmet needs.

You must work at all times within yourown competence and professional Code of Conduct (GMC).

The hours for this role are 1-2 days (2-4 sessions) per week.

Main duties of the job
  • You will undertake a variety of duties, either remotely offering support and advice to System providers regarding vulnerable patients in Community as part of an MDT or F2F as part of our preventative visiting service.
  • You will undertake best interest discussions with patients and family members and help co-ordinate a response to support frail people to stay home whenever possible.
  • You may be needed to visit a complex frail patient at home or undertake telephone consultations.
  • Make professional, autonomous decisions in relation to presenting problems.
  • Assessing the health care needs of complex patients with undifferentiated and undiagnosed problems, highlighting which services would support them best to stay in their home setting and independent whenever possible.
  • Cover the clinical management of patients as appropriate including initiating investigations, reviewing results, making referrals to other providers/services as appropriate.
  • Record clear and contemporaneous computerised consultation notes to agreed standards.
  • Compile and issue computer-generated acute and repeat prescriptions.
  • Prescribe generically whenever appropriately and in accordance with NICE guidance and Good Prescribing Practice (as defined by British National Formulary guidance).

Work with and support other members of the MDT, includingbut not limited to social workers, therapists, social prescribers, voluntarysupport workers, frailty nurses, dieticians, UCR paramedics and other FrailtyGPs.

Promote the application of standards and codes of Practiceset by the Royal College of General Practice, General Medical Council,Department of Health, British Geriatrics Society and local NHS trust.

Participate in national frailty training and maintain highvolume of CPD in Frailty related conditions with the aim of obtaining theDiploma in Geriatric Medicine.

Other Responsibilities:

Involve and contribute to the mentoring, supervision andtraining of the broad multi-professional clinical team.

Be aware of and comply with all relevantpolicies/guidelines, e.g. prescribing, confidentiality, data protection, healthand safety.

Commitment and contribution to achievement of the highestpossible quality standards.

Commitment to life-long learning and audit to ensureevidence-based best practice.

Contribute to evaluation/audit and clinical standard settingwithin the organisation.

Attendance at regular in-house governance and educationalmeetings and training as well as events at other agencies as appropriate.

Assist with the training of other staff members.

Contribute to the development of computer-based patientrecords.

Contribute to the summarising of patient records andRead-Coding patient data.

Take responsibility that any equipment either clinical orotherwise, used when working, is taken due care of and locked away at the endof every working session.

Build and maintain relationships with other healthcareprofessionals, including GPs, GP surgery personnel, care home personnel and CCGpersonnel etc.

Safeguarding:

Everyone within Nimbuscare has a responsibility for, and iscommitted to, safeguarding and promoting the welfare of Adults-at-risk(vulnerable adults), children and young people and for ensuring that they areprotected from harm.

Be fully aware of and understand your duties andresponsibilities arising from the Childrens Act 2004 and Working Together toSafeguard Children 2018, in relation to child protection and safeguardingchildren and young people.

Be fully aware of and understand your duties andresponsibilities arising from the DOH Policy No Secrets (2000), and theprinciples of safeguarding as they apply to Adults-at-risk (vulnerable adults)in relation to your work.

Recognise the types and signs of abuse and neglect.

Ensure the Nimbuscare Safeguarding Lead is alerted to, andkept fully informed of, any concerns you may have in relation to safeguardingadults, children and young people.
