Position Objective:The Hospice Care RN is responsible for managing a caseload of home care and facility-based hospice patients. This includes creating and evaluating care plans, providing direct patient care, and ensuring interdisciplinary coordination. The RN will also assist facilities in identifying prospective hospice patients, perform in-service training, and participate in an on-call and rotating weekend/holiday schedule.
Key Responsibilities:Patient Care and Case Management:
- Develop individualized care plans in collaboration with patients, families, and physicians.
- Regularly review and update care plans to meet patients' evolving needs.
- Conduct ongoing clinical, functional, and emotional assessments to identify issues and track progress.
- Coordinate care with interdisciplinary teams through effective communication and case conferences.
- Provide direct therapeutic interventions as needed, with appropriate physician approval.
Supervision and Delegation:
- Supervise and delegate tasks to Home Health Aides and Licensed Practical Nurses.
- Develop and revise care plans for paraprofessional services in line with agency policies.
Documentation and Compliance:
- Complete and submit all required clinical documentation accurately and in a timely manner.
- Utilize laptop systems to document care plans, clinical notes, and other records while on-site and transmit data twice daily.
Facility and Community Support:
- Manage hospice patients within assigned facilities and conduct community hospice visits as needed.
- Facilitate case management meetings and provide in-service training to staff, patients, and families.
On-Call and Rotational Duties:
- Participate in night shift rotations with the hospice team.
- Cover weekend and holiday shifts as part of a rotating schedule.
Performance Expectations:- Timely coordination of patient care plans and interdisciplinary communications.
- Effective delegation and supervision of ancillary personnel.
- Accurate and prompt submission of documentation in compliance with agency standards and state/federal regulations.
- Commitment to providing compassionate and patient-centered hospice care.
Qualifications:- Active RN license in Colorado.
- Prior experience in hospice or palliative care nursing is preferred.
- Strong organizational and critical thinking skills.
- Excellent communication and teamwork abilities.
- Proficiency with electronic health records and mobile documentation systems.
