Job Description: Asa Home Health RN, you will be responsible for providing comprehensive nursingcare to patients in their homes. You will work closely with patients, theirfamilies, and other healthcare professionals to develop and implementindividualized care plans.

Key Responsibilities:
  • Conduct initial and ongoing assessments of patient health status.
  • Develop and implement personalized care plans based on patient needs.
  • Administer medications and treatments as prescribed by physicians.
  • Monitor patient progress and adjust care plans as necessary.
  • Educate patients and their families on disease management, medication administration, and other healthcare topics.
  • Coordinate with other healthcare professionals to ensure continuity of care.
  • Maintain accurate and timely documentation of patient care.

Qualifications:
  • Current RN license in the state of North Carolina.
  • Minimum of 1-2 years of nursing experience, preferably in home health or a related field.
  • Strong clinical skills and the ability to work independently.
  • Excellent communication and interpersonal skills.
  • Compassionate and patient-focused approach to care.
  • Valid driver's license and reliable transportation.
