JOB SUMMARY: A member of the patient care team within the Hospice department and directly accountable to the Director of Hospice and is delegated an assignment by the Patient Care Manager or PCC. The position requires that the individual assist the professional nursing staff by performing specific nursing functions related to patient care under the supervision of a registered nurse and for the secretarial aspects of patient care and the environmental management of the work unit. The individual must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served. Demonstrate knowledge of the principles of growth and development and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs. Provide reliable assistance to the members of the team, including nurses, managers, and physicians. Provide the care needed. Demonstrate good customer service skills when interacting with patients, visitors, and co-workers. Perform other job duties as assigned. Help ensure that the unit runs efficiently. Prepare documentation ongoing accurately and timely according to policies and procedures. Assist with team meetings. CNA participates in in-service and competency programs as applicable.

  • Demonstrates ability to bathe patient in shower or bed using appropriate water temperature, preventing falls, and maintain privacy.
  • Demonstrates ability to transfer patients safely utilizing safe body mechanics for yourself and patient.
  • Demonstrates ability to document in the EMR according to policies and procedures.

QUALIFICATIONS:

Education/Skills: High School Diploma/GED and completion of an approved Nurse Aide training course required.

Work Experience: Six months of Nurse Aide work experience preferred. Computer experience required

Mental Demands: Ability to perform repetitive tasks, cooperate with others in a positive

manner, take orders and follow directions precisely, read and write, communicate

effectively and positively, maintain rapport in all situations, and adapt to varied

socioeconomic levels and environments.

CERTIFICATION/LICENSURE: During department orientation, required to successfully complete a 75-hour certification program as a Home Health aide. Completion of a Nurse Aide training program from an approved school preferred. If no prior certificate, must successfully complete a 75-hour certification program as a Home Health Hospice aide and demonstrate competency in Nurse Aide skills test during orientation. CPR certification required by the completion of clinical orientation. Current Mississippi driver's license and proof of automobile insurance are also required.
