Overview: Join Our Team as a Triage Team Coordinator We are seeking a compassionate and dedicated Triage Team Coordinator to serve our hospice patients and their families. In this role, you will perform clerical/scheduling activities that facilitate agency operations related to employee and patient schedules. These activities include but are not limited to coordination of customer/employee schedules, generating orders, frequency tracking, schedule changes/updates, visit assignment, employee availability changes/ updates, personnel associations, manage staff productivity, continuity of care for the patient, manage overtime, discontinue orders, schedule according to authorization and assure compliance with conditions of participation/ regulatory requirements and internal processes as it relates to scheduling. Upload documents timely and accurately to EMR system. Provides clerical support and assists with projects as delegated. Answers emails and faxes and processes accordingly. Assisting triage clinical team on shifts with processing referrals and scheduling. Qualifications: A heart to serve patients and families and a passion for providing the best possible care Education: Associate degree preferred Experience: Minimum of 1-year experience in a health care setting. Scheduling and office manager experience preferred Required: Reliable transportation.
