Trinity Criminal Background Check must be completed
Trinity Education Verification must be provided
Trinity EPLS report must be attached
Trinity GSA/OIG report must be attached
Trinity NSO report must be attached
Trinity System Access Form must be completed

American Heart Association certification is required for all direct patient care personnel. Attach scanned copy of certification (with expiration date) to candidate's record in TrinityVMS. Must include an image of the front of the card as well as an image of the back of the card.
