RADIUS RULE: TRAVELER MUST BE 50 MILES OUTSIDE THE FACILITY. Social Worker-Aspirus Wausau, Contract Length: 13 weeks
• Start Date: ASAP
• Shift Requirements: 800-430 pm M-F, every 7th weekend rotation
• On Call/Call Back Requirements: NA
• Required Certifications to be Attached upon Submittal: Masters Degree in Social Work (MSW) and WI License
• Minimum Experience: 1 yrs experience in social services and counseling in a hospital acute care setting with a diverse population
• Charting Experience : epic
• Unit Information : Med Surg and Critical Care Clusters- all inpatient units- Layered Model working in tandem with RN Team for Discharge planning
• Patient Ratio 8-20 per day based on acuity of patients
