  • Comprehensive surgical services, 24-hour ED services, home health & hospice agency, advanced wound healing center, cardiac rehabilitation center, 2 cath labs, one day surgery center, medical and radiation oncology.
    Job Requirements
  • Required certifications prior to start: None ACM or CCM preferred
  • Years of Experience: 2 years
  • Must Have Skills/Experience: Discharge planning in the acute hospital setting.
  • Preferred Skillset/Experience: ACM or CCM preferred. Creating discharge plans, based on insurance benefit, knowledgeable of utilization reviews, coordinating services to prevent readmission. Knowledgeable of CMS regulation and Virginia Medicaid.
    Schedule Information
  • Orientation is bi-weekly on Tuesdays
  • Shift: 9-5:30pm
  • Weekly Guaranteed Hours: 40
  • Weekend Hours: 8-4:30p
  • Call Requirements: None

  • Equipment: Laptop, two monitors, desk and phone provided by facility
