Job Details
The Registered Dietitian is responsible for developing and implementing the WelbeHealth nutrition services program working in close collaboration with the Interdisciplinary Team (IDT).
Deliver nutrition services such as assessments and counseling to participants.
Function as a liaison between WelbeHealth and food and supplement service providers.
Develop menus and ensure compliance with all regulations pertaining to kitchen and meal service delivery.
Collaborate effectively with colleagues and stakeholders to promote WelbeHealth values, team culture, and mission.
Conduct nutrition assessments to determine nutrition needs, preferences, and goals of participants.
Participate in IDT meetings to develop participant care plans.
Deliver and document direct care nutrition interventions according to physician orders and care plans.
Implement and monitor special diets and meal intake.
Provide nutrition counseling to participants and families.
Oversee menu planning and meal delivery program to ensure safety and regulatory compliance.
Oversee coordination of supplements as ordered.
Provide staff training on meal delivery program as needed.
Support smooth care transitions between settings with PACE Physician and IDT members.
Support intake and enrollment process for prospective participants.
Remain alert to input from team members, participants, and caregivers and update IDT on changes.
Be legally authorized to perform in position location and act within scope of authority.
Willingness and ability to work varied schedule including evenings, nights, and weekends.
Complete all required documentation timely and accurately.
Protect privacy and maintain confidentiality of company procedures and information.
Follow WelbeHealth policies and participate in quality improvement activities, staff training, and meetings.
Communicate regularly with supervisor and team regarding workload and priorities.
Complete all mandated trainings and occupational health screenings timely.
Exercise flexibility in assignments as business needs evolve.
Other duties as assigned.
Job Requirements
Minimum of four years' experience managing nutrition services in a similar setting.
Demonstrated ability to mentor and develop team members.
Minimum of one year of experience with a frail or elderly population.
Excellent organizational and communication skills.
Ability to work independently with minimal supervision.
Demonstrated ability to prioritize in a fast-paced environment.
Experience and competency working with people from diverse backgrounds and cultures.
Commitment to unlocking the full potential of vulnerable seniors.
Schedule Information
Willingness and ability to work a varied schedule that may include evenings, nights, and weekends.
