Education:
Doctor of Physical Therapy (DPT) degree from an accredited program. Certification Requirements:
Current Physical Therapist license in the state of Oklahoma.
Basic Life Support (BLS) certification. Experience:
Minimum of 3 years of experience as an Physical Therapist
