Summary

As a Physical Therapist in a Skilled Nursing Facility, you will be essential in providing high-quality patient care and rehabilitation services. Reporting to the Rehabilitation Director, you will utilize your core skills in anatomy, physiology, and medical terminology to develop effective care plans. Your expertise in discharge planning will ensure smooth transitions for patients returning home or to community settings. Additionally, your premium skills in acute care and pediatrics, along with relevant experience in outpatient and geriatrics, will enhance the recovery process and improve patient outcomes in our facility.

Duties
Conduct thorough evaluations of patients to assess their physical condition and functional abilities.
Develop individualized treatment plans based on patient assessments, goals, and needs.
Implement therapeutic interventions including manual therapy techniques, exercise programs, and modalities to promote healing and recovery.
Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
Educate patients and their families on rehabilitation processes, home exercise programs, and lifestyle modifications to support recovery.
Collaborate with healthcare professionals to coordinate comprehensive patient care, including discharge planning and follow-up services.
Maintain accurate documentation of patient evaluations, treatment plans, progress notes, and discharge summaries in compliance with regulatory standards.
Stay current with advancements in physical therapy practices and participate in ongoing professional development.

Qualifications
Valid Physical Therapist license
Strong patient care and assessment skills
In-depth knowledge of anatomy and physiology
Proficiency in medical terminology and physical therapy techniques
Experience with discharge planning and care plans
Familiarity with acute care settings and geriatrics
Background in outpatient, inpatient, and home health environments
