Job Summary: Assesses and evaluates patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, develops a treatment plan, performs interventions aimed at improving and enhancing the patient's well being, and evaluates the patient's progress. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements list must be representative of the knowledge, skills, minimum education, training, licensure, experience, and/or ability required.
