ROLE ABILITIES REQUIREMENTS: Regulatory/Educational Requirements
• High School Diploma or equivalent.
• Current State Physical Therapy Licensure, graduate of an approved school of Physical Therapy or graduate of accredited college or university with a certificate in Physical Therapy.
• Two (2) or more years of previous experience in all aspects of physical therapy preferred.

Outpatient Care
Summary:
We are seeking a compassionate and skilled Outpatient Physical Therapist to join our dedicated rehabilitation team at Southwest Health System in Cortez Colorado. In this role, you will work with a diverse patient population, providing therapeutic interventions and rehabilitation programs designed to help individuals improve mobility, manage pain, and achieve optimal functional outcomes. The ideal candidate is passionate about patient care, has excellent clinical skills, and is eager to collaborate in a supportive, multidisciplinary environment.
Key Responsibilities:
- Patient Evaluation: Conduct comprehensive initial evaluations to assess patients' physical conditions, medical history, and rehabilitation needs. Develop individualized treatment plans based on evidence-based practices and patient goals.
- Therapeutic Treatment: Provide one-on-one physical therapy treatments, including manual therapy, exercise prescription, therapeutic modalities, and education to optimize physical recovery and mobility.
- Patient Education: Educate patients and their families about injury prevention, self-management techniques, and home exercise programs.
- Progress Monitoring: Regularly assess patient progress, adjust treatment plans, and document outcomes using the clinic's electronic health record (EHR) system.
- Collaboration: Work closely with physicians, other therapists, and healthcare providers to coordinate care and ensure holistic treatment plans.
- Documentation & Compliance: Maintain accurate, timely, and complete patient records in compliance with HIPAA and clinic policies.
- Community Outreach: Participate in patient education programs and community health initiatives to promote wellness and preventative care.
- Professional Development: Stay current with the latest physical therapy research, techniques, and best practices. Attend continuing education programs and team meetings as required.
Qualifications:
- Licensing: Active state licensure as a Physical Therapist.
- Experience: Minimum of 1-2 years of experience in outpatient physical therapy preferred. New graduates are welcome to apply.
- Skills & Competencies:
- Strong knowledge of musculoskeletal, neurological, and orthopedic conditions.
- Proficiency in a wide range of physical therapy techniques and modalities.
- Excellent communication and interpersonal skills to interact effectively with patients and team members.
- Ability to develop and implement individualized treatment plans tailored to patient needs.
- Strong organizational skills and attention to detail for documentation and scheduling.
