Shift Notes- 9p - 7:30am, Mon night through Sun. 7 on 7 off schedule is optional
Required Certifications- BLS
Guaranteed Hours- 72 hours per two weeks
Charting System- Allscripts - Sunrise
Hours: 7 on/7off beginning on Monday evenings 9pm-730am. Will be working with 1 other pharmacist.
License/Certifications/Special Requirements: Possesses current Illinois Pharmacist License or Letter of Authority from Department of Professional Regulation in State of Illinois.
Unit Notes-
This position is responsible to help patients utilize the medications effectively and assure the right patient receives the correct drug in the proper amount at the right time and all doses are appropriate to treat the patient's condition.
a.) Produces and distributes products with accuracy and efficiency.
b.) Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose. Recommends changes or alternatives as needed.
c.) Uses pharmacokinetic concepts to prescribe appropriate drug therapies requiring dosage adjustments per hospital protocol.
d.) Recommends appropriate lab tests and monitors values to determine if the attainment of therapeutic efficacy without the development of toxicities or side effects.
e.) Detects and reports suspected adverse drug reactions accurately and in a timely manner.
This position requires providing services to one or more age populations, including neonate through geriatric, in a manner that demonstrates an understanding of the functional/developmental age of the individual served.
