Job Description & Requirements

Requirements:
- COVID Vaccination:
- Dose date must be on or after 10/3/23
- If requesting an exemption, official form with provider/religious leader signature required
- Approved exemptions require masking at all times on-site
- License: Active Massachusetts Pharmacist license
- Education: Doctoral Degree in Pharmacy (PharmD) - required, non-negotiable
Experience:
- 3+ years acute care clinical pharmacy experience - required
- IV admixture & parenteral chemotherapy admixture experience - preferred
Other:Must be able to work independently Must supervise support personnel when necessary Traveler must have their own vehicle BLS (AHA) certification required
