Occupational Therapist II Location: Newport Beach, CA Facility: Hoag at Home Schedule: Full-time, Monday-Friday, 8:30 AM-5:30 PM Job Summary: The Occupational Therapist II provides in-home therapy services, addressing physical, cognitive, and sensory-perceptual aspects to enhance patients' daily living skills. This role involves patient assessments, treatment planning, therapeutic interventions, and caregiver education. Responsibilities: Evaluate patients' functional abilities and develop individualized treatment plans. Provide therapeutic interventions to improve mobility, independence, and quality of life. Conduct regular reassessments and modify treatment plans as needed. Educate patients and caregivers on therapy techniques and home exercises. Oversee Occupational Therapy Assistants and Rehabilitation Aides. Maintain accurate documentation in compliance with professional standards. Participate in team meetings, staff education, and community outreach programs. Requirements: Minimum 18 months of experience as an Occupational Therapist in an acute healthcare setting. Experience with geriatrics, neurology, orthopedics, cardiopulmonary, and general medicine. California Occupational Therapist license and BLS certification required. Home health experience and ability to travel for 5-6 home visits per day required. Familiarity with Kinnser EMR preferred. Bilingual skills are a plus.
