How long is the contract assignment? 13 weeks 2) Schedule of assignment hours? M - F, 8am - 5pm, with call at least once a month 3) Will this candidate be visiting patients in care facilities or only in patient's homes? Patient homes 4) Will you want to hire the contractor at the end of the contract or is this truly a contract assignment? Potential to hire 6) What region are you in? Home health or Hospice? Home health 7) What's creating the request? Part-time RN is moving to another care center 8) To which location will this person report for assignment, full address, Including the OU (Location Code) Thomaston, GA 3347 H Crawley St Thomaston, GA 30286 Upson, Meriwether and Pike Counties May have to see patients in Spalding and Lamar counties also Please confirm above location is correct and list any other territories that are to be covered by the candidates: 9) How many miles per week will the consultant travel? 30/40 a day? Weekly? Varies 10) What are the weekly units of productivity expectations? 30 points 11) What EMR systems will the consultant be using: HomeCare HomeBase, Oasis, Outlook 12) Will there be any on call? If so, how much? On call at least once a month, call is for Upson, Meriwether, Pike, Spalding and Lamar counties. Starts on Friday and goes through the following Thursday. 13) Do you want the candidate to have home health experience? Yes
