Job Description:- Job Title: Registered Nurse (RN) - Home Health (Travel Position) Facility: SSM Health at Home - Reedsburg Location: Reedsburg, WI Shift: 08:00 AM to 04:30 PM Scheduled Hours: 40 hours per week Key Responsibilities: Provide direct patient care in the home setting, including assessments, wound care, medication administration, and other home health services. Collaborate with interdisciplinary healthcare teams to ensure comprehensive care plans are followed and updated. Monitor patients' progress and communicate with physicians and family members regarding changes in patient condition. Ensure proper documentation and charting using EPIC and Oasis systems. Educate patients and their families on health management and self-care techniques. Requirements: Experience: Minimum of 1 year of Adult Home Health experience required. Certifications: Current BLS (Basic Life Support) certification required. Licenses: RN state license or any other compact state license required. Traveler Restrictions: No local candidates within 70 miles of Reedsburg, WI.
