Job Summary: We are currently seeking a dedicated and reliable Home Health Registered Nurse (RN) for a 13-week contract in Portage, Wisconsin. This is a night shift position, working 8-hour shifts providing one-on-one skilled nursing care in the comfort of the patient's home. The ideal candidate will have a strong clinical background and the ability to work independently.
Key Responsibilities: - Deliver compassionate, evidence-based nursing care in a home health setting
- Perform physical assessments and document patient progress during night shifts
- Administer medications, treatments, and monitor vital signs as ordered
- Communicate and coordinate with the patient's care team, including physicians and family members
- Ensure documentation is accurate and completed in a timely manner
- Respond to changes in patient condition promptly and appropriately
- Maintain a safe, clean, and supportive environment in the patient's home
Requirements: - Active RN license in Wisconsin (or compact license)
- Minimum 2 year of RN experience (Home Health experience preferred)
- Night shift availability for the full 13-week assignment
- Valid driver's license and reliable transportation
- Current BLS/CPR certification
- Strong time management and documentation skills
- Ability to work independently in a home care environment
