Role Overview:
As a Case Manager RN, you will serve as a crucial link between patients, providers, and the healthcare system-ensuring smooth transitions of care, optimizing resource utilization, and improving patient outcomes across the care continuum.

Key Responsibilities:

Comprehensive Care Coordination:

  • Collaborate with physicians, social workers, and the interdisciplinary team to create timely and cost-effective care plans.
  • Coordinate transitions to the appropriate level of care, aiming to prevent unnecessary admissions or readmissions.
  • Lead and support the healthcare team in executing care plans aligned with patient goals and clinical needs.

Patient Assessments & Risk Evaluation:

  • Conduct initial and ongoing assessments using patient interviews, chart reviews, and collaboration with clinical staff.
  • Identify patients at high risk for readmission and initiate preventative interventions.
  • Screen for palliative care needs and facilitate appropriate referrals.

Utilization Management:

  • Review documentation and clinical data to support appropriate level of care (LOC) determination.
  • Work directly with physicians to ensure compliance with clinical documentation requirements.
  • Monitor length of stay (LOS) and engage in early discharge planning to reduce delays.

Care Transitions & Discharge Planning:

  • Develop and document individualized transition plans in collaboration with patients, families, and care teams.
  • Facilitate follow-up care, ensure completion of discharge paperwork, and verify compliance with Medicare and state regulations.
  • Identify and address social determinants of health that may impact care outcomes.

Communication & Team Collaboration:

  • Participate in patient rounds and interdisciplinary discussions to update and refine care plans.
  • Communicate with Financial Counselors regarding insurance coverage and patient needs.
  • Act as a liaison between internal teams, third-party payers, and external support agencies.

Compliance & Best Practices:

  • Stay current on CMS, Joint Commission standards, and hospital policies.
  • Ensure timely and accurate documentation in EHR and specialized case management software.
  • Identify opportunities for process improvement, resource optimization, and enhanced patient experience.
