Role Overview:
As a Case Manager RN, you will serve as a crucial link between patients, providers, and the healthcare system-ensuring smooth transitions of care, optimizing resource utilization, and improving patient outcomes across the care continuum.
Key Responsibilities:
Comprehensive Care Coordination:
- Collaborate with physicians, social workers, and the interdisciplinary team to create timely and cost-effective care plans.
- Coordinate transitions to the appropriate level of care, aiming to prevent unnecessary admissions or readmissions.
- Lead and support the healthcare team in executing care plans aligned with patient goals and clinical needs.
Patient Assessments & Risk Evaluation:
- Conduct initial and ongoing assessments using patient interviews, chart reviews, and collaboration with clinical staff.
- Identify patients at high risk for readmission and initiate preventative interventions.
- Screen for palliative care needs and facilitate appropriate referrals.
Utilization Management:
- Review documentation and clinical data to support appropriate level of care (LOC) determination.
- Work directly with physicians to ensure compliance with clinical documentation requirements.
- Monitor length of stay (LOS) and engage in early discharge planning to reduce delays.
Care Transitions & Discharge Planning:
- Develop and document individualized transition plans in collaboration with patients, families, and care teams.
- Facilitate follow-up care, ensure completion of discharge paperwork, and verify compliance with Medicare and state regulations.
- Identify and address social determinants of health that may impact care outcomes.
Communication & Team Collaboration:
- Participate in patient rounds and interdisciplinary discussions to update and refine care plans.
- Communicate with Financial Counselors regarding insurance coverage and patient needs.
- Act as a liaison between internal teams, third-party payers, and external support agencies.
Compliance & Best Practices:
- Stay current on CMS, Joint Commission standards, and hospital policies.
- Ensure timely and accurate documentation in EHR and specialized case management software.
- Identify opportunities for process improvement, resource optimization, and enhanced patient experience.
