-RN, BSN Preferred - will take ADN with extensive COMPLEX Care Mgmt experience
-BLS, CCM preferred but not required
-Must be RN Case manager with 5 years RECENT Acute Care Management experience (hospital ONLY) with NO LESS THAN 1 YR previous experience in a Level 1 and/or large academic Medical Center hospital.
-Will cover all Adult Inpatient units throughout the hospital - must have experience in multiple inpatient units. Level 1 ER experience is a PLUS
-EPIC Charting Proficiency is REQUIRED
-Ensure Epic and Trauma levels are included in the work history.
Common types of patients & job duties: Medically and socially complex patients that require some "out of the box" thinking to get them moved. Experience in Discharge planning for complex patient populations, no UR piece to the role.
In addition to being medically complex discharges patients may have additional needs complicating the discharge such as psych diagnosis, requiring trach and vent management or be sex offenders. Mississippi is not a Medicaid expansion state so does not cover LTAC or skilled which increases the challenge in placing the complex patients upon discharge.

Case load/ patients per day/consults per day 15-20
EMR EPIC
InterQual criteria or Milliman experience not required but need to have general understanding of discharge readiness
Potential to plan discharge to a variety of setting including home, home with home health, SNF, LTAC, hospice, etc.
Will work in tandem with Social Work team
UMClient has contracts with 5 local facilities with 10 beds reserved at each facility to assist with the complex placements, however specific criteria must still be met. Criteria has a limitation on financial assistance for only those uninsured or underinsured.
UMClient is the only level 1 trauma hospital in the state so will receive a variety of complex patients and those with no other hospital options.
Will directly report to the Social Work Manager. Chief of Care Coordination is also providing department oversight as needed.
No Weekend requirement

Pharmacy for DC Planning? Will consult with Pharmacy as needed
Department Specific Orientation 2 weeks (will include general facility orientation) Will work alongside CM team so will never be working alone.
