Previous acute inpatient case management experience, 3-5 years case management experience, strong computer skills, Epic EMR experience, BLS certification from AHA. Typical Case Ratio is 1:21 - 25, will not be meeting with every patient - this is decided based on individual risk factors. Schedule: 8:30AM - 5PM rotating schedule, weekend availability required. Temp to perm option available but not required.
