Shift: 8:00 AM - 4:30 PM Description: Primary Location: Aspirus at Home Crystal Falls Shift requirement: 8 hours days. Roughly 8a-430p, but depending on census there may be flexibility to start and end times. Hours: 40 per week Weekend/Holiday: NA On-call: NA Travel Requirements: 20-25 patients weekly, 4-5 visits per day on average. It can vary from 25-60 miles per day depending on the territory, patient census, staff availability, etc Mileage can be up to and over 100 miles as it is a large territory. Mileage reimbursement at current government rate. Required Certifications: AHA BLS, Active MI and WI RN license, Valid Driver's license, Proof of Insurance - OASIS cert a PLUS. Minimum experience: 2+ years home health experience, preferred. Must have travel experience Charting Experience: Oasis required. Epic required Unit Information (Bed size, Case type, Patient ratio): Oasis admissions, all time points, episode management, dr calls and coordination of care, documentation in EMR timely, clinical competencies in wound, IV, ports, Lab draws, chronic disease management Hard worker, Time management, Resilient and Manages Change well, Social candidates, Dependable and Accountable, Compassionate, Positive Attitude REQUIRED EDUCATION:Associate Degree or Certificate PREFERRED EDUCATION:Bachelor's Degree HOLIDAY ROTATION REQUIREMENTS:N/A
