Case Manager (RN) - CommonSpirit Health Location: Zone 3 - Denver, CO (OrthoColorado Hospital, Longmont United Hospital, St. Anthony Hospital, St. Anthony North Hospital)


Shift: 8-hour day shifts (8:30 AM - 4:30 PM)


Weekends Required: Yes (2 weekend days per month)


On-Call Requirements: Yes


Job Responsibilities
  • Review and assess patient information for admission in compliance with policies, documenting assessments in case management software.
  • Conduct comprehensive evaluations of patients' physical, psychosocial, cultural, and spiritual needs through interviews, record reviews, and consultations with healthcare teams.
  • Oversee utilization management processes and ensure alignment with established guidelines.
  • Facilitate discharge planning in collaboration with patients, families, and interdisciplinary teams, making necessary referrals and arrangements.
  • Participate in performance improvement initiatives through concurrent chart reviews and involvement in clinical effectiveness teams.
  • Document case management actions in EMR and confirm treatment goals with healthcare teams.
  • Utilize evidence-based guidelines and criteria to support care plans and optimize patient outcomes.
  • Communicate treatment goals and best practices to healthcare teams, ensuring adherence to established guidelines.
  • Coordinate and evaluate resource utilization in alignment with patient care plans, addressing variances with interdisciplinary teams.
  • Lead family conference meetings and interdisciplinary rounds to streamline care and discharge processes.
  • Collaborate with social workers, homecare coordinators, ambulatory care case managers, and utilization reviewers to enhance service delivery and prevent delays in discharge.
  • Maintain updated referral resource lists and track variances in care plans for process improvement.
  • Identify appropriate patient status and level of care, leveraging regulatory guidelines and physician consultations.
  • Manage interactions with third-party payers, securing approvals for patient stays and addressing denial management challenges.
  • Perform utilization reviews, ensuring compliance with UM Plan and minimizing variance days.
Qualifications
  • Education: Associate Degree in Nursing (required); Bachelor's Degree in Nursing (preferred)
  • Licensure: State RN license or RN license from a participating NLC state (required)
  • Certification: American Heart Association Basic Life Support (BLS) (required)
  • Experience: Minimum 1 year of nursing or case management experience
  • Skills: Working knowledge of regulatory requirements and accreditation standards (preferred)
Float Requirements
  • May be required to float to any CommonSpirit location within 60 miles of the original assignment or an identified float zone.
  • Float assignments may include duties beyond original job requirements per CommonSpirit policy.
