Position Summary: Nurse Case Manager (NCM I)
Location: Not specified Shift: 8:00 AM-4:30 PM (may consider 3x12s)
On-Call: No Float Requirement: No
Requirements: - Experience: Minimum 2 years RN experience required
- Education: BSN required
- Certifications: BLS required, Case Management certification preferred
- License: Active SC or compact RN license required at start
- Preferred Experience: Prior travel leadership, Pediatrics, Epic EMR
Role Overview: The Nurse Case Manager I reports to the Manager of Case Management and Care Transitions and works under the guidance of the NCM Team Leader. This role:
- Coordinates care plans to support clinical outcomes and safe discharges
- Manages patient care across the continuum to promote efficiency
- Acts as a clinical expert in discharge planning, resource use, and case management
