Position Title: RN Case Manager - Stroke & Med/Surg Unit
Schedule:40 hours/week
Shift: 10-hour Days, 4 shifts per week (no set day off)
- Works every 6th weekend
- No major holidays required
Unit:
Menino 7 West - Stroke and Medical-Surgical (MS/T) Patients
Required Experience:
- Minimum 3 years of experience in acute inpatient care
- Strong background in current Med/Surg standards of practice
- Skilled in assessing physical and mental impairments
- Deep understanding of illness characteristics and treatment planning
- Familiarity with insurance benefits, plan contracts, eligibility, and regulatory requirements
Preferred Experience:
- 5+ years in Utilization Management (UM) and Care Management (CM)
- EPIC experience strongly preferred
Required Education/Certification:
- Bachelor of Science in Nursing (BSN)
Preferred Certification:
- CCM (Certified Case Manager) or related certification
Role Summary:
The Case Manager utilizes a collaborative, patient-centered process to assess, plan, implement, monitor, and evaluate health services required to meet individual patient needs. This includes managing the length of stay, ensuring appropriate utilization of resources, and supporting cost-effective, high-quality outcomes. The Case Manager is responsible for determining the medical necessity and appropriateness of admissions using InterQual criteria and Medicare inpatient lists. The role manages a caseload of 18-22 patients on the Menino Campus.
