Job Description;
Parking Reimbursement: $250/month
- Schedule:
- 4x10-hour day shifts (40 hours/week)
- No fixed day off
- Works every 6th weekend
- No major holiday shifts required
- Unit: Menino 7 West - Med/Surg & Telemetry
- Patient population includes stroke and general medical-surgical cases
- Required Experience:
- Minimum 3 years in Med/Surg nursing
- Strong understanding of acute inpatient care and Med/Surg practice standards
- Skilled in assessing both physical and mental impairments
- Familiar with treatment protocols, benefit plans, and eligibility criteria
- Preferred Experience:
- 5+ years in Utilization Management (UM) and Case Management (CM)
- Proficiency with EPIC EMR
- Certifications:
- Required: BSN
- Preferred: CCM (Certified Case Manager) or similar certification
- Role Overview:
- Manages care for 18-22 patients through a collaborative, interdisciplinary approach
- Focuses on ensuring quality, cost-effective outcomes
- Key responsibilities include:
- Developing and managing individualized care plans
- Monitoring resource utilization and length of stay
- Determining medical necessity using InterQual and Medicare inpatient guidelines
