Case Manager (RN) - Med/Surg - Menino 7 West Schedule: AM Shift: 10-hour shifts 40 hours/week (4x10s) No set day off Every 6th weekend rotation No major holidays required Unit Details: Department: Menino 7 West (MS/T) Patient Population: Stroke and Med/Surg patients Caseload: 18-22 patients Campus: Menino Campus Role Summary: The Case Manager (CM) will utilize a collaborative process to assess, plan, implement, monitor, and evaluate services required to meet each patient's health needs. The CM will: Promote quality, cost-effective outcomes Manage length of stay (LOS) Ensure efficient utilization of hospital resources Determine medical necessity and appropriateness of admission using InterQual screening and Medicare inpatient list Required Experience: Minimum 3 years of experience Extensive background and knowledge in: Current Medical-Surgical patterns of practice Acute inpatient care Methods for assessing physical and mental impairment Illness characteristics and expected treatments Benefits, plan contracts, eligibility issues, and insurance requirements Preferred Experience: 5+ years in Utilization Management (UM) and Case Management (CM) EPIC proficiency Required Certifications: BSN (Bachelor of Science in Nursing) Preferred Certifications: CCM (Certified Case Manager) or related certification
