RN Case manager for Princeton Home Health South Team.
Role: RN Case Manager - Home Care/Home Health (LOCAL PREFERRED)
Schedule: M-F 8:30AM - 5:00PM (1 weekend per month)
Responsibilities: 4-6 home visits per day in assigned territory (South NJ/Princeton area)
EMR/Systems: Epic, Oasis
Required Experience: 2+ years of home health (non private duty) or community based experience
Licensure: RN (NJ, Compact)
Contract: 13 weeks
- Orientation will be at Plainsboro/Princeton site (1-2 days tops)
- Weekend requirement 1 weekend in 4
