Job Description:
We are seeking a dedicated Home Health Registered Nurse for a 13
•week contract, working 40 hours per week, primarily during daytime hours.
Responsibilities:
• Provide exceptional nursing care to patients in their homes.
• Complete documentation using HCHB and OASIS, with same
•day submission.
• Manage a weekly caseload of 20
•25 cases, achieving 5
•7 productivity points daily.
• Attend weekly in
•person office meetings.
• Offer flexible care within 8 am
• 5 pm, accommodating patient needs.
Requirements:
• Active RN License and BLS certification.
• At least 2 years of home health experience.
• Proficient in HCHB/OASIS, with experience in Oasis E post
•January 2023.
• Ability to manage a diverse caseload and maintain productivity.
• No more than a 1
•year gap in relevant experience.
