Unit Notes - Case Management Department Experience Requirements: RN NY license must - Minimum: 1 year of inpatient case management experience
- Preferred: Critical care background
Required Certifications:- Basic Life Support (BLS) - Required
- PRI & SCREEN Certifications - Preferred

- Each certification typically takes a few hours to complete

Role Summary: - Focus: Inpatient discharge planning
- Typical Units: Medical-Surgical (MS) or Telemetry
- Collaboration: Works in partnership with a Social Worker (SW); interdisciplinary rounding with physicians/teams
- Responsibilities Include:
- Assessing discharge needs and making appropriate referrals
- Coordinating placement to home, skilled nursing facilities (SNFs), or shelters
- Completing shelter packets, hemodialysis referrals, wound care (KCI) referrals
- Utilizing a binder with relevant discharge resources
- Key Skills Required:
- Independent functioning, strong critical thinking, time management
- Positive interpersonal skills and adaptability in dynamic care settings
Caseload & Workload: - Average Floor Size: 33-40 patients
- Typical Staffing:
- Standard Floors: 1 Case Manager (CM) & 1 SW
- Pilot Floors: 1 CM & 2 SWs
- Minimum Caseload Capacity Required: Ability to manage at least 20 patients
Technology & Tools: - Electronic Medical Record (EMR): Allscripts
- Preferred Knowledge:
- CarePort, MCG, and/or InterQual
- Prior Utilization Review (UR) experience is a plus
- Note: UR functions (e.g., insurance approvals, transport) are handled by a separate department
Scheduling & Shifts: - Regular Hours: Monday-Friday, 8:00 AM - 4:30 PM (7.5-hour shift with 1-hour lunch)
- Breaks: Per facility policy; full lunch breaks supported by management
- Weekends/Holidays: Rare, only with manager approval; caseloads may vary during these shifts
Dress Code: - Attire: Business casual or scrubs (excluding Royal Blue and Maroon; neutral tones preferred)
- Lab Coat: Provided by the facility
Orientation: - Initial Training:
- 2 days of hospital orientation (includes EMR training)
- 1-2 weeks of shadowing/preceptorship, duration based on performance and proficiency
