Shift: 8hr Day Shift (40) Shift Information: M-F; On-call, Call Back Requirements: None Weekend Rotation: None Holiday requirements: None Floating requirements: Within service area; possible to float to neighboring areas JOB DESCRIPTION Case Manager can expect to see 20-30 patients per week. Patients typically have multiple comorbidities and can include (but not limited to): CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, fall, fracture, infections, cellulitis, post-surgical, wound care, ostomy care, labs, IV infusion, etc. Support Team: Manager/Supervisor, Lead, Scheduler Any color scrub acceptable but must be appropriate/professional. EPIC/Computer charting, Laptop, Sapphire IV pumps. Traveler must provide their own transportation to/from all patient visits. CANDIDATE MUST HAVE: - 1 year Home Health RN Experience - active state RN license and BLS (AHA) Skills required: Wound care, including wound vacs, pleural drains, ostomy care; peripheral blood draws, central line care, IV administration through multiple devices including elastomeric and IV pumps, excellent assessment skills, experience with computer documentation "We are an equal opportunity employer.
