Job Description:
• Title: Home Health Case Manager
• Start Date: 04/14/2025 (Must be within 4 weeks)
• Shifts: Monday-Friday; 08:00-16:30
• Duration (weeks): 13 weeks
• Guaranteed Hours: 40
• RTOS: No more than 5 days of RTO accepted.
• Patient Ratios: 20-30 patients per week. Expectation is for 5- 6 routine visits per day.
• Coverage area(s): West Berlin, Ripon, Wautoma, Omro, Winneconne, and Oshkosh. Requirements:
• 1 year Home Health RN experience.
• Active BLS (AHA).
• Active RN license (WI or compact).
• Experience with wound care (wound vacs, pleural drains, OASIS, ostomy care), peripheral blood draws, central line care, and IV admin through multiple devices (elastomeric, IV pumps).
• Excellent assessment skills.
• Experience with computer documentation.
• Reliable transportation, active driver's license, and insurance.
