Job Summary: Medical Technician position at Saint Joseph Health System Mishawaka, Indiana.
Shift: 2200 - 0630 with every other weekend rotation and holiday. No call.

BLS certification required for all direct patient care personnel.
License/Certificate Verification must be dated within 30 days of start.
Fit Test required prior to clearance.
Trinity 9 Panel Drug Screen must be completed within 30 days of start date, preferred within 14 days.
Trinity Code of Conduct Form must be signed and on file.
Trinity I9 must be completed by the agency.
Trinity Confidentiality of Patient and Proprietary Information Agreement must be signed and on file.
Trinity Integrity and Audit Services Form must be on file.
Trinity Color Vision must be verified.
COVID-19 Vaccine proof required.
MMR vaccination proof required.
Hepatitis B vaccination proof required.
Influenza Vaccine proof required.
Varicella vaccination proof required.
Tdap vaccination proof required.
Physical/Health Statement must be provided.
Trinity Profile including resume and references required.
Trinity Conflict Of Interest Disclosure Form must be signed and on file.
Trinity Criminal Background Check must be completed and dated within 30 days of start.
Trinity Education Verification must be attached.
Trinity EPLS report must be attached and dated within 30 days.
Trinity GSA/OIG report must be attached and dated within 30 days.
Trinity NSO report must be attached and dated within 30 days.
Trinity System Access Form must be completed and uploaded one week before start.
2 Step PPD OR IGRA (Quantiferon/Tspot) must be completed and uploaded.

American Heart Association certification is required for all direct patient care personnel. Attach scanned copy of certification (with expiration date) to candidate's record in TrinityVMS.
