Job Summary:
We are currently seeking experienced and compassionate LPNs/LVNs for a Home Health position in Raleigh, NC. This contract role involves visiting patients in their homes to provide skilled nursing care in accordance with physician orders. The ideal candidate will have prior home health experience and be comfortable working independently in the field using Homecare Homebase (HCHB) software.
Roles and Responsibilities: - Deliver direct patient care in a home setting under the supervision of an RN or physician.
- Administer medications, perform wound care, and monitor patient health.
- Complete documentation accurately and timely in Homecare Homebase (HCHB).
- Educate patients and families on health care needs, medication instructions, and other home health protocols.
- Participate in care planning and communicate any changes in patient condition to the care team.
- Maintain confidentiality and adhere to professional standards and ethical guidelines.
- Participate in weekly on-call rotation.
Qualifications: - Licensure: Active LPN/LVN license in the state of North Carolina - Required
- Certifications: Current CPR certification - Required
- Experience:
- 1-2 years in a clinical care setting - Required
- 1-2 years of home health experience - Required
- Familiarity with Homecare Homebase (HCHB) - Required
- Other:
- Valid driver's license and reliable transportation with active insurance - Required
Education: Graduate of an accredited Licensed Practical Nursing Program or School of Vocational Nursing.
