JOB: DISCHARGE LPN START DATE 5/19 Nights Every Other Weekend (Fri/Sat) Must have at least two completed travel assignments. OR LPN License and AHA BLS required Work with patients who are medically ready for discharge but are experiencing delays to various barriers in the Medically Ready for Discharge Unit under the supervision of the RN. Must have experience:
• Acute care experience
• Knowledge of discharge planning processes
• Basic nursing skills
• Patient assessment
• Documentation
• Basic patient education techniques
