Job Summary
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in a clinical setting, assisting in various aspects of patient management and ensuring adherence to established healthcare standards. Duties
- Administer medications and treatments as prescribed by physicians.
- Assist with patient assessments, including vital signs and medical history documentation.
- Participate in discharge planning and ensure patients understand their care plans.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Maintain accurate medical documentation in compliance with clinic standards.
- Provide support for behavioral health needs, ensuring a holistic approach to patient care.
- Utilize electronic health record systems such as Epic and Athenahealth for efficient documentation and communication.
- Assist in sonography procedures as needed, ensuring patient comfort and safety.
- Engage in case management activities to facilitate continuity of care for patients.
- Perform airway management when necessary, following established protocols.
Qualifications
- Valid Licensed Practical Nurse (LPN) license in the state of practice.
- Previous experience in a clinical or healthcare setting preferred.
- Proficiency in medical documentation practices and familiarity with electronic health records (EHR).
- Knowledge of discharge planning processes and case management principles.
- Experience or training in behavioral health is a plus.
- Strong interpersonal skills with the ability to communicate effectively with patients, families, and team members.
- Ability to work collaboratively within a multidisciplinary team environment.
