Job Summary
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in a clinical setting, assisting in various aspects of patient management and ensuring adherence to established healthcare standards. This role requires strong clinical skills, effective communication, and a commitment to patient-centered care.

Duties

  • Administer medications and treatments as prescribed by physicians.
  • Assist with patient assessments, including vital signs and medical history documentation.
  • Participate in discharge planning and ensure patients understand their care plans.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Maintain accurate medical documentation in compliance with clinic standards.
  • Provide support for behavioral health needs, ensuring a holistic approach to patient care.
  • Utilize electronic health record systems such as Epic and Athenahealth for efficient documentation and communication.
  • Assist in sonography procedures as needed, ensuring patient comfort and safety.
  • Engage in case management activities to facilitate continuity of care for patients.
  • Perform airway management when necessary, following established protocols.

Qualifications

  • Valid Licensed Practical Nurse (LPN) license in the state of practice.
  • Previous experience in a clinical or healthcare setting preferred.
  • Proficiency in medical documentation practices and familiarity with electronic health records (EHR).
  • Knowledge of discharge planning processes and case management principles.
  • Experience or training in behavioral health is a plus.
  • Strong interpersonal skills with the ability to communicate effectively with patients, families, and team members.
  • Ability to work collaboratively within a multidisciplinary team environment.
  • Commitment to maintaining high standards of patient care and safety.
