Shift: Days (Monday-Friday, typically 8 a.m.-5 p.m.)
Position Overview:
We are seeking an experienced and compassionate LVN/LPN for a home health assignment. In this role, you will provide care to patients in their homes, focusing on individualized, one-on-one care. You will be assigned to a specific territory, with minimal floating required and only in cases of low census. This is an excellent opportunity for an LVN/LPN who values flexibility, patient-focused care, and working independently in the community.
Requirements:
- Active LVN license in the state of Massachusetts
- Current BLS certification
- Minimum of 2 years of recent home health experience
- Experience with OASIS required
- Homecare Homebase (HCHB) experience preferred but not required
- No on-call requirement
- Must be comfortable with self-scheduling and patient autonomy
- Must be open to floating to nearby territories if necessary (rare)
