What You'll Do:
- Evaluate and treat residents with a wide range of conditions
- Develop individualized treatment plans based on resident goals and needs
- Collaborate with an interdisciplinary team to support resident care
- Document services in accordance with facility and regulatory standards
- Educate residents, families, and caregivers on therapy goals and progress
What We're Looking For:Active state Physical Therapist license
Prior experience in a SNF or geriatrics setting preferred
Strong communication and teamwork skills
Compassionate, dependable, and adaptable in a fast-paced environment
