Mental Health and/or Healthcare experience required. Oncology/serious illness experience preferred.
Position is specifically for our genitourinary cancers team that handles a high volume of prostate cancer patients.
Dedicated ambulatory social worker providing clinical social work services to patients and their families.
Position calls for clinical psychosocial assessment and intervention, including crisis response, individual, couples and family consultation, therapy, and counseling, and short-term bereavement support.

Position is specifically for our genitourinary cancers team that handles a high volume of prostate cancer patients.

Must be AHA. Red Cross certs may be accepted but we must request approval on a case by case basis. If the candidate has a cert from another organization, then they may be able to complete AHA version onsite, but must receive facility approval in advance. AHA instructor cards are acceptable.
Health Questionnaire - form must be completed by candidate prior to start and signed on the last page. Candidate must bring this completed form with them to the health screening appointment.

Confidentiality Agreement - must be signed by candidate prior to start
Background Attestation (Time Sensitive.) - completed by the CL. This must be completed ASAP after the CORI results are received, as it is required for facility's initial onboarding.
Intellectual Property Agreement (Time Sensitive.) - IPA must be completed by hand, signed with a wet signature and uploaded within 72 hours of signing contract.
