Discharge Planner Case Manager Shifts: (8A-430P) 5 x 8 Days On call/Weekend Requirement: no on call requirement; weekends included in rotation (every 5th or 6th weekend); holiday coverage as needed Floating Requirement: no floating requirement Local Rate withing 60 miles of facility Requirements: Will accept RN Case Manager Discharge Planner or Licensed Social Worker - Social worker must be eligible for LMSW/LGSW/LCSW -they must have taken and passed the exam already - DC RN Licensure - WILL ACCEPT PENDING DC LINCEUSNRE WITH PROOF OF APPLICATION (Aya paid open license project) OR candidate can hold LMSW/LGSW/LCSW - BLS - 2 years of experience - Must have acute hospital discharge planning experience - Skills required: Emergency Department, Case management This facility requires that all employees have the Flu vaccine Business Casual-Associate s appearance should be clean, neat and professional. NO SCRUBS
