Job Title: Social Services Specialist Job Specialty: Master of Social Work (MSW) - Case Management Job Duration: 13 weeks Shift: Day Shift, 5x8-Hour (08:00 - 16:30) Guaranteed Hours: Not specified Experience: Minimum years required not specified License: Post-MSW license, such as the licensed social work associate independent clinical, issued in Washington Certifications: BCLS required Must-Have: - Masters in Social Work - Experience in case management and discharge - Open to having EITHER an Associate clinical license or LCISW - Ability to utilize InterQual Criteria and Milliman Guidelines Job Description: - Open to both MSW Care Managers and RN Care Managers. - Role involves coordinating care, assessing patients, and planning discharges. - Utilization management experience is helpful, but not required. - The average daily census ranges between 18-20. - The candidate must be able to work with acute hospital settings, patients with communication problems, community programs, and advanced directives. - Coordination with home care services and referrals to rehab and long-term care facilities is essential. - Effective documentation according to the plan of care and understanding of coverage criteria is required. - Familiarity with CMS, CPT coding and billing, HIPAA, ICD 10 coding, and related regulatory requirements is necessary. - Required to use charting systems such as Epic and Cerner. - Must be amenable to floating and fulfill responsibilities across different units as needed. - A minimum of two supervisory references covering one year (or six months for CNAs) within the last three years is required. - Completion of pre-start and annual modules is mandatory.
