Discharge Planner or LCSW/LMSW
Shifts: (8A-430P) 5 x 8 Days
On call/Weekend Requirement: no on call requirement; weekends included in rotation (every 5th or 6th weekend); holiday coverage as needed
Floating Requirement: no floating requirement
Local Rate withing 60 miles of facility
Requirements:
Will accept RN Case Manager/Discharge Planner or Licensed Social Worker
Social worker must be eligible for LMSW/LGSW/LCSW -they must have taken and passed the exam already
DC RN Licensure - WILL ACCEPT PENDING DC LINCEUSNRE WITH PROOF OF APPLICATION (Aya paid open license project) OR candidate can hold LMSW/LGSW/LCSW
BLS
2 years of experience: discharge planning to include: admission criteria review, benefits eligibility, care coordination, conducting needs assessments and order DME; experience with home health, hospice placement, LTAC, nursing home placement, rehab placement, Acute hospital experience; prior charge experience preferred
- completion of at least 1 travel assignment
Regulatory Skills required: CMS, HIPPA, JCC; workers compensation preferred

Scrubs: Uniform is business casual, no scrubs professional appearance
