Job Details - ALL APPLICANTS MUST HAVE 1 year Certified Home Care experience.
- 1 year Case Management Experience.
- Ability to travel to cases as assigned.
- Weekend rotations are once every 6 weeks.
- As a Home Health RN, responsibilities not limited to assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families.
- Patient care is delivered within organizational philosophy, policy and standards of community health practice.
- Also required to perform admission processes and case management responsibilities in accordance with MGB Home Care's policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.

Additional Details - Submit a color photo taken in past 6 mos. No selfies, no filters. Plain Background. Symmetrical headshot, shoulders up, look straight/no tilted head. No eyeglasses.
- MVR check to be run by background check company within 45 days of start. Required for all home care positions.
- Primary Source Verification of professional license and/or certification, as required for the position. Within 30 days of start date and updated upon expiration.
- N95 - within 365 days to start date and updated annually.
- Within 30 days of start. Required by MA DPH for all employees.
- A completed primary COVID-19 vaccination series that is approved by the FDA or the World Health Organization (WHO) OR an up-to-date COVID vaccine if a primary series was never received or completed is required.
- Documentation of two measles vaccines, two mumps vaccine, and one rubella (German measles) vaccines or two MMR vaccines.
- Vaccine required during flu season Sept 15th - April 30th. Document must be uploaded as PDF.
- Must have documentation of TB skin test screening within 3 months of start date and updated if >6 month break.
