    Job Details
  • Conduct Home Health Nursing assessments to determine nursing, personal care, and equipment needs in the home.
  • Regularly re-evaluate participant nursing needs.
  • Initiate and revise the plan of care based on participant status and needs.
  • Develop care plans incorporating therapeutic, preventative, and rehabilitative nursing actions.
  • Include participant and family in care planning process.
  • Deliver and document home health nursing interventions as per care plans.
  • Instruct participants, family, and caregivers on disease processes, self-care, and prevention strategies.
  • Remain alert to input from team members and update interdisciplinary team on participant condition changes.
  • Work with providers and interdisciplinary team to manage care transitions between settings.
  • Identify and implement discharge planning needs.
  • Instruct, supervise, and evaluate home health aide care biweekly.
  • Participate in end-of-life care coordination and support.
  • Provide backup coverage to other nursing functions.
  • Perform duties consistent with Nursing Practice Act and California Code of Regulations.
  • Willingness and ability to work varied schedules including evenings, nights, weekends, and overtime.
  • Complete required documentation timely and accurately.
  • Protect privacy and maintain confidentiality of company procedures and participant information.
  • Follow company policies, participate in quality improvement activities, staff training, and meetings.
  • Communicate regularly with supervisor and team regarding workload and priorities.
  • Complete mandated trainings, education, and occupational health screenings timely.
  • Exercise flexibility in assignments as business needs evolve.
    Job Requirements
  • Nursing knowledge and skills to treat frail, elderly participants and manage complex clinical situations.
  • Ability to initiate venous access for IV hydration or lab draws.
  • Experience administering IM, SC, or transdermal injections and providing wound care.
  • Highly motivated, self-directed, able to make sound decisions in emergency situations.
  • Excellent clinical, organizational, and communication skills with seniors, families, and interdisciplinary teams.
  • Ability to work independently with minimal supervision.
  • Demonstrated ability to prioritize in a fast-paced environment.
  • Experience and competency working with diverse backgrounds and cultures.
  • Commitment to supporting vulnerable seniors.
  • Preferred: Minimum two years nursing experience in clinical setting with frail or elderly population.
  • Preferred: Home health agency experience.
  • Preferred: Bilingual English/Spanish.
