Key Responsibilities:
• Deliver physical therapy services in a home setting.
• Collaborate on individualized care plans.
• Document patient progress using HCHB and Oasis.
• Conduct Start of Care assessments.
Schedule:
• 13
•week contract, 40 hours/week
• Day shifts, Monday to Friday
Qualifications:
• Valid PTA License
• BLS certification
• 1+ year home health experience
• Proficient in HCHB and Oasis
