Evaluates and treats all referred Physical Therapy patients according to physician orders.
Performs and supervises physical therapy programs in the homes of HFM Home Health Care clients.
Performs initial patient evaluations including administering appropriate tests/measures, identifies impairments and functional limitations.
Establishes an initial plan of treatment in conjunction with the patient/caregiver.
Selects and administers treatment strategies based on current physician orders and established treatment plan(s).
Delegates appropriate tasks and supervises the PTA or HHA.
Identifies functional improvement and goal attainment on a regular basis, supported by documented subjective and objective changes.
Updates treatment plans and goals per department standards.
Completes documentation following department standards and timeframes, reflecting skilled intervention and supporting therapeutic interventions as reasonable and necessary.
Achieves productivity per department standards and completes job duties efficiently and effectively.
Complies with applicable rules and regulations in Home Health Care.
Demonstrates accuracy with OASIS and adheres to the National Patient Safety Standards.
Demonstrates responsible utilization of resources for each patient and actively participates in department PI activities.
