Job Description:
We are seeking a skilled Home Health Physical Therapist to provide in
•home care. This role requires expertise in HCHB/OASIS documentation and a commitment to patient
•centered care.
Key Responsibilities:
• Provide quality physical therapy to home health patients.
• Complete same
•day documentation using HCHB.
• Manage a caseload of 20
•25 cases weekly, achieving 26
•28 productivity points.
• Attend weekly in
•person office meetings.
• Be flexible to float as needed.
Requirements:
• Valid PT License and BLS Certification.
• 2+ years of home health experience.
• Proficient in HCHB/OASIS, with Oasis E experience post
•January 2023.
• Efficient caseload management.
• Reside within 30 minutes of the territory or secure nearby housing.
Schedule:
• 13
•week contract, 40 hours per week.
• No on
•call or overtime.
• Pre
•approved time off required.
Additional Information:
• 2
•3 days of orientation and team introduction.
• Flexible hours, branches open 8 am to 5 pm.
