- Trinity MVR (Motor Vehicle Report) dated within 30 days of start
- Trinity Skills Checklist for clinical contractors
- 2 Step PPD OR IGRA (Quantiferon/Tspot)
- COVID-19 Vaccine proof required
- MMR vaccination proof required
- Hepatitis B vaccination proof required
- Influenza Vaccine proof required
- Varicella vaccination proof required
- Tdap vaccination proof required
- Physical/Health Statement documentation required
- Trinity Profile including resume and references
- Trinity Conflict Of Interest Disclosure Form signed
- Trinity Criminal Background Check dated within 30 days of start
