$500 Travel Allowance Tour of Duty: The Contractor is responsible for performance, thirty (30) to forty (40) hours per week, between the hours of 7:30am and 5:00pm, Monday through Friday, excluding lunch breaks, Federal holidays, administrative facility closures, approved absences, and verifiable emergencies. Qualifications: A nutritionist under this contract will be a credentialed Registered Dietician (RD) or Registered Dietician Nutritionist (RDN) with a current and unrestricted license to practice as an RD or RDN in the United States, the District of Columbia, Puerto Rico, or a territory of the United States. The nutritionist shall remain in good standing with the licensing board throughout the contract term. Nutritionist Duties: The Contractor shall provide nutritionist services in delivery of patient care to the Indian Health Service. Specific Tasks include the following: 5.1.1 Develop, coordinate, and implement quality nutrition consultation services and medical nutrition therapy (MNT) with an emphasis on promoting health, preventing disease, and disease management. 5.1.2 Provide individuals, families, groups, service unit staff, and institutions with advice, consultation, training, education, and guidance in regards to diets, menus, recipes, meal plans/schedules, nutritional care plans, weight loss, and the prevention and management of chronic diseases. 5.1.3 Provide nutrition expertise in the planning, delivery, implementation, and evaluation of outpatient and community nutrition care. 5.1.4 Conduct nutritional and physical activity assessments of patients. 5.1.5 Counsel patients and families in nutrition principles, MNT plans, food selection and economics, adapting plans to the individual's lifestyle and resources. Demonstrate an understanding of various indigenous principles regarding food, nutrition, and food as medicine to provide culturally competent and relevant care to active beneficiaries of the service unit. Provide patients with written instructions regarding exercise and nutrition goals. 5.1.6 Review laboratory values, medications, and other objective and subjective data points to define and adjust MNT and treatment plans accordingly. 5.1.7 Work effectively and harmoniously with care teams, colleagues and coworkers, and with AI/AN patients and their families. Demonstrate cultural competency, awareness of and sensitivity to the cultures and traditions of American Indians and Alaskan Natives. 5.1.8 Coordinate with patients' care teams. Offer guidance and support to team members. Promote a collaborative and whole health approach to care through effective communication with primary care providers and other service unit staff. 5.1.9 Participate in continuous quality and performance improvement processes, including both the development and implementation of quality and performance improvements. 5.1.10 Participate in accreditation and licensing processes. 5.1.11 Work with interpreters as necessary to provide services to customers. 5.1.12 Complete facility and department orientations. 5.1.13 Complete all training required by IHS and service unit policy. 5.1.14 Participate in clinical rounds, care conferences, and meetings. 5.1.15 Comply with Indian Health Service immunization requirements. 5.1.16 Document individual and group visits, including proper documentation of nutrition prescriptions, nutritional recommendations, education topics, and evaluation of each patient progress toward goals utilizing the Nutrition Care Process (NCP), endorsed by the Academy of Nutrition and Dietetics (AND), in each patient's individual health record within 72 hours of the patient contact. All health information generated in the performance of the contract shall meet or exceed the established standards of the service unit, including but not limited to timeliness, legibility, accuracy, and content standards. 5.1.17 Respond to notifications requesting the completion of visit records within 72 hours. 5.1.18 Provide program evaluation data monthly, to include number of patients served, frequency, topics, methodology, impact, and other metrics as needed.
